How to apply (medical and other exceptional circumstances)

Submit an application form via Hallpad.

Under the ‘Medical Circumstances’ section of the form, enter your circumstances as they pertain to your accommodation application - i.e. how your circumstances affect what kind of accommodation you should be offered.

Print your application form and send it to your GP along with the GP assessment form (available to download from Hallpad).

Your GP is required to review the information you have declared in your application form, approve this and return a completed GP assessment form to the Disability and Wellbeing Service.

Once submitted, the Disability and Wellbeing Service will meet with the Residential Services Office to review your requirements so that we can determine the most appropriate offer for you and whether any adjustments should be made to your room/hall.

We will then communicate a decision on your application.
Please Note: Although offers of accommodation may be made in advance of the completed GP assessment forms being returned, these offers are provisional.
