Please see the details set out below for the information that we will require about your disability. If you are not sure that your documentation is going to be acceptable to the School, please discuss this with us and we will be happy to look at it for you. 

We will also ask you to complete a Disclosure Form if you wish us to put in place adjustments for you in the School. If you wish to make an appointment online for the first time, please complete an Appointment Registration Form and email it DWS.

Dyslexia, dyspraxia and specific learning difficulties

  • a diagnostic assessment report conducted AFTER you were 16
  • in English (translated if not originally in English)
  • from an appropriately qualified practitioner (usually a chartered psychologist or a Specialist Teacher with a curent Practicing Certificate) 

Physical and sensory impairments, and long-term medical conditions, including mental health difficulties

  • a completed LSE DWS Medical Information form from your GP or specialist
  • in English (translated if not originally in English)
  • from a suitably qualified practitioner, medical service or a relevant institution or service
  • if you have Attention Deficit / Hyperactivity Disorder (AD/HD), Asperger’s Syndrome (AS) or High Functioning Autism (HFA), we will need a letter or report from the Psychiatrist or Clinical Psychologist who diagnosed this condition, as well as a completed Medical Information form.

There are separate arrangements in place if you need Individual Examination Adjustments.
